Birth Certificate / Proof of Citizenship
High School Diploma / G.E.D
Illinois Drivers License
Illinois State Firefighter II Certification
Illinois Department of Public Health- Paramedic License ( or proof of being enrolled in an approved paramedic course. Candidates
must be licensed prior to employment)
Valid Candidate Physical Ability Test (CPAT) card, issued within the calendar year.
